Preservé Breast Augmentation by Motiva

woman bikini

If you’ve been researching breast augmentation, you’ve probably noticed the conversation is changing. For decades, “boob job” meant the same basic operation: a long incision, the chest muscle lifted off the rib cage, and a recovery measured in weeks of soreness. Preservé® by Motiva is a less invasive technique that flips that script. Instead of cutting through breast tissue and muscle, it gently separates and protects them, so the implant slides into a precise, custom-made pocket. Dr. Sarah Mess is a board-certified plastic surgeon based in Columbia, Maryland, offering Preservé to patients across Howard County, Baltimore, Annapolis, and the greater Washington, D.C. metropolitan area who want a beautiful, natural result with less downtime and less disruption to the body that got them here.

The PRESERVE technique, combined with Motiva implants, represents a new standard in breast augmentation — one built around protecting your body, not just transforming it. By minimizing tissue disruption and pairing the procedure with ultrasound-guided nerve blocks, we reduce anesthesia requirements and help you heal faster, with less discomfort. This is precision surgery designed with your recovery in mind.”

What Is Preservé by Motiva?

Preservé is a breast tissue preservation system developed by Motiva (Establishment Labs). Instead of using traditional surgical dissection, your surgeon uses two purpose-built tools, the Motiva Channel Separator and the Motiva Inflatable Balloon, to create the space for the implant without cutting through the breast’s natural support structures. Nipple sensation, milk ducts, Cooper’s ligaments, and the pectoralis muscle are designed to stay intact.

The Channel Separator and Inflatable Balloon are registered with the U.S. FDA and are used in the United States with FDA-approved Motiva SmoothSilk® Ergonomix® or Round implants, which received Premarket Approval (PMA) in September 2024.1,2 That’s an important detail. Some of the clinical data you’ll see online comes from international studies using a related implant (Ergonomix2) that is not approved in the U.S. Dr. Mess will walk you through what’s evidence-based here vs. abroad, so you can make a fully informed choice.

How Preservé Works, Step by Step

In a traditional breast augmentation, a surgeon dissects and detaches tissue with handheld instruments to create the implant pocket. Preservé works differently. Here’s what Dr. Mess actually does in the operating room:

1
Precision Measurement

Before surgery, Dr. Mess takes detailed measurements of your breast width, height, and tissue thickness. Those numbers determine the exact implant size and where the pocket needs to sit so your result fits your anatomy, not a template.

2
A Small, Hidden Incision

Once the area is numbed, a roughly 2.5–3 cm incision is placed in the inframammary fold (the natural crease beneath the breast). It’s tucked where a bra band sits, so the scar is discreet.

3
The Channel Separator

Instead of cutting, the Motiva Channel Separator slides through that small opening and creates a tunnel by gently parting the tissue. Think of it as opening a path between strands of a rope rather than slicing the rope in half. Blood supply, nerves, and muscle attachments are largely left alone.3

4
The Inflatable Balloon

Next, the Motiva Inflatable Balloon is inserted and slowly expanded to shape a pocket that’s a precise match for your implant. It does the “stretching” the surgeon’s hands would normally do, but in a controlled, gradual way.

5
No-Touch Implant Placement

Finally, the Motiva SmoothSilk Ergonomix implant is inserted using a no-touch sleeve, which keeps the implant from contacting skin and reduces bacterial exposure. The incision is closed with meticulous, layered sutures.

For appropriate candidates, the entire procedure can often be performed under local anesthesia with sedation, instead of general anesthesia. Many patients walk out of recovery the same day with no grogginess, and no chest “pulled apart” feeling.

Benefits of Preservé Breast Augmentation

When the procedure goes the way it’s designed to, here’s what patients can expect, in plain English:

  • Preserved nipple sensation. Nerve pathways that travel through breast tissue stay intact.4
  • Muscle preservation. Your pectoralis muscles are not cut or detached, which means no “animation deformity” when you flex.1
  • Smaller incisions. Typical Preservé incisions are 2.5–3.5 cm, often shorter than standard augmentation incisions.5
  • Faster recovery. Most patients are off prescription pain medication within a few days and back to desk work inside a week.6,7
  • Implant stability. A three-year international clinical study reported 0% inferior malposition (“bottoming out”) in Preservé patients.8
  • Lower inflammatory response. Motiva’s SmoothSilk nano-surface is engineered to minimize the body’s foreign-body response, which may translate to lower capsular contracture rates.9
  • Less anesthesia exposure. When appropriate, the procedure is performed awake with sedation, not general anesthesia.

A note on honesty: Preservé doesn’t levitate. It’s still surgery, done in an accredited facility with full safety protocols. Results vary, complications are possible, and implants aren’t lifetime devices. Dr. Mess will tell you all of this straight — including if she thinks a different approach would actually get you a better result.10

Are You a Candidate for Preservé?

Preservé is fantastic, but it’s not a fit for everyone. You may be a great candidate if you are:

  • Considering breast augmentation for the first time (primary augmentation)
  • Considering augmentation with a lift (primary mastopexy-augmentation)
  • Looking for a subtle, natural enhancement in the range of one to two cup sizes
  • Hoping to preserve nipple sensation and the option to breastfeed in the future
  • Healthy, a non-smoker (or willing to quit), with realistic expectations
  • Frustrated with the recovery stories you’ve heard from friends and want less downtime

You may be better served by a different approach if you:

  • Want a very large size change (Preservé works best with Motiva Ergonomix implants typically up to about 315 cc)
  • Need significant revision surgery for capsular contracture, severe asymmetry, or implant malposition
  • Have anatomical features (very thin tissue, prior radiation, complex chest wall shape) that require a different surgical plan
  • Are looking for a heavily augmented “done” look that requires more pocket dissection

The only way to know for sure is a one-on-one consultation. Dr. Mess will tell you honestly whether Preservé is your best option or whether another technique would give you a better result.

The Implant: Motiva SmoothSilk® Ergonomix®

smoothsilk motiva implant
Preservé pairs the technique with a specific implant: Motiva SmoothSilk Ergonomix, which received U.S. FDA approval in September 2024. A few things that make this implant interesting:

  • It adapts to body position. Lying down, it spreads into a round shape. Standing up, gravity gives it a soft, natural teardrop silhouette. That’s the “ergonomic” part.11
  • It feels soft. Its proprietary ProgressiveGel and TrueMonobloc® shell allow it to behave more like natural tissue.
  • The surface matters. SmoothSilk is a nano-textured surface engineered for low inflammation and good tissue integration without the aggressive macro-texture that’s been associated with rare complications in other implant brands.12
  • BluSeal® barrier. A visible barrier layer that helps confirm the implant shell is intact at the time of placement.
  • Strong clinical data. Five-year FDA clinical trial data published in Aesthetic Surgery Journal (2025) demonstrated favorable safety and effectiveness outcomes for Motiva SmoothSilk implants in primary and revisional augmentation.13

What to Expect: Consultation, Surgery, and Recovery

Your Consultation

You’ll meet with Dr. Mess (not a coordinator alone) to talk through goals, look at sizing options, review your medical history, and examine your breast anatomy. You’ll see Motiva implant samples in person, hear honest pricing, and get the time you need to ask every question on your list.

The Day of Surgery

Most Preservé procedures take about 60–90 minutes. You’ll go home the same day. Many patients are surprised by how not dramatic the experience is: a breast augmentation with the option of no general anesthesia (when appropriate), no chest tightness, and a small incision that’s already covered when you wake up.

Recovery, Realistically

Day 1–2

Mild soreness, often managed with over-the-counter pain relief and a short course of prescription medication if needed

Day 3–5

Most patients are off prescription painkillers and back to light daily activities

Week 1

Many return to desk-based work

Weeks 2–4

Most return to gentle exercise; heavier lifting and chest workouts are introduced gradually

Months 2–3

Swelling fully resolves and final shape settles

Long-term

Periodic follow-ups; breast implants are not lifetime devices and routine surveillance is recommended14

Frequently Asked Questions

Is Preservé FDA-approved?

The Motiva SmoothSilk Ergonomix and Round implants used with Preservé received U.S. FDA Premarket Approval in September 2024 (PMA P230005). The Motiva Channel Separator and Inflatable Balloon that define the Preservé technique are FDA-registered devices used with FDA-approved implants in the United States. The Ergonomix2 implant used in some international Preservé studies is not currently approved in the U.S.15

How is Preservé different from a “regular” breast augmentation?

Traditional augmentation uses handheld instruments to dissect and detach breast tissue and often part of the pectoral muscle. Preservé uses the Channel Separator to gently part the tissue and the Inflatable Balloon to expand a pocket, so blood supply, nerves, and muscle attachments are largely preserved.

Is Preservé performed awake?

For appropriate candidates, yes, the procedure can be performed under local anesthesia with sedation rather than general anesthesia.³ Your candidacy is determined at consultation.

Will I still be able to breastfeed?

Because Preservé is designed to preserve glandular tissue and milk ducts, most patients can still breastfeed afterward. No breast surgery can guarantee future breastfeeding success, however, so it’s important to discuss your family plans during consultation.

How big can I go with Preservé?

Preservé is designed for natural, one-to-two-cup-size enhancements. Motiva Ergonomix implants used in Preservé in the U.S. are typically chosen up to roughly 315 cc. If you’re hoping for a more dramatic size change, Dr. Mess will discuss other options.

How long is recovery?

Most patients are off prescription pain medication in a few days, back to desk work within a week, and cleared for full activity by four to six weeks. Final shape settles over two to three months.

Will my scar be visible?

Incisions are typically 2.5–3 cm and placed in the inframammary fold (the crease under the breast), where they are hidden by a bra or bikini top.

Is Preservé safe?

Preservé combines FDA-approved Motiva implants with FDA-registered preservation instruments. Like any breast implant surgery, it carries risks including capsular contracture, infection, rupture, and the possibility of reoperation. Breast implants are not lifetime devices. A board-certified plastic surgeon should perform the procedure in an accredited facility.

How much does Preservé cost in Maryland?

Pricing depends on implant choice, anesthesia, facility fees, and whether other procedures are combined. You’ll receive a transparent, written estimate at the time of consultation.

Where can I get Preservé breast augmentation near me in Maryland?

Dr. Sarah Mess performs the Preservé by Motiva technique at her practice in Columbia, Maryland. She sees patients from throughout Howard County, Baltimore, Annapolis, Ellicott City, Rockville, and the greater Washington, D.C. and Northern Virginia area.
